A mission-critical clinical telemetry platform engineered to transmit real-time ICU diagnostics and laboratory sensor streams across 120+ clinical sites. Replaced legacy polling infrastructure with an event-driven local-first architecture built for extreme reliability under adverse network conditions.

Clinics in regional areas suffered from 45-second polling latency and frequent packet collisions, causing patient biometric monitoring systems to desynchronize during critical triage handoffs. Network interruptions routinely resulted in buffered telemetry being dropped upon reconnection.

We designed a local-first, distributed edge architecture. Telemetry is written immediately to a high-speed SQLite embedded cache on the local clinical gateway, encrypted with AES-256-GCM, and streamed to central clusters via bidirectional WebSockets with automated backoff and state reconciliation.

Edge gateways run lightweight Go ingestion services that multiplex sensor feeds into binary WebSocket frames. The central ingestion layer uses Redis Streams to distribute incoming streams across a multi-tenant TimescaleDB cluster for continuous biometric time-series storage. The web frontend runs on Next.js with Web Workers offloading client-side decryption.

High-contrast, distraction-free clinical UI optimized for rapid diagnosis under high-stress environments. Visual status indicators prioritize biometric anomalies using IEC 60601-1-8 medical alarm hierarchy.

Built with Next.js App Router, Go edge ingestion daemon, TimescaleDB for time-series persistence, Redis Streams pub/sub, WebSockets, and Web Workers. Full mTLS encryption between all edge nodes and central VPC.

Median telemetry propagation latency plunged from 45,000ms to 82ms (a 99.8% reduction). Over 1.4 million vitals logged with 0% data loss across 12 months. Achieved 100% compliance score on HIPAA and SOC2 Type II audits.
